So for those of y'all who don't know who we are, we are Kids on the Block. We're an educational puppet group. So we travel the state and we present puppet programs to K-6 grade students.
We currently have 10 program topics for mental health, child abuse prevention, substance use prevention, all

Okay, so this is one of our puppets. This is Steven. He's in our Child Abuse Prevention Programs. And, you know, a lot of people might think that we're just fun little programs to go in and educate or to entertain children, but our programs reach deeper into education. And they really do have a lasting impact on children. And so I want to share a couple little stories following our programs. So I have Steven here with me today. He is in our child abuse prevention program. He was with me the day that we did some child abuse prevention programs at an elementary school for children in K through 6th grade. Following that program, about a week after that, the guidance counselor called us and he told me that they had two children that disclosed they were being abused. They both came separately and disclosed the abuse. But come to find out, those two children were from the same family and they were both being sexually abused by their mother. those children were removed from the home and then when we went back to that school a little later I got an update that those children had been removed from the home and the children were now with a different family they before had behavioral issues when they were at school those behavioral issues were gone and the children were making good grades and thriving so that's the kind of impact that our programs can have on children they can help children feel safe and get out of dangerous situations. Another story that we had from this school year was we were presenting our substance use prevention program at a school for fourth and fifth grade children. That program obviously teaches children about substances, how harmful they are to your body, and obviously we don't want children to try them. But we also teach that they can always talk to an adult they feel safe with. And so following that program, we had a child come forward and she told one of our puppeteers that her parents were arguing all the time and she was really upset and she didn't know what to do and she also then went on to say that she had started stealing she didn't want to steal she knew it was wrong but she didn't know how to stop and so our puppeteers were able to get her to the counselor so the counselor could help her figure out what she needed to do for herself and you know how to help her family And so our puppets really do have an impact. They reach children where they are, because our puppets are different characters, but they each play a vital role in helping children understand what to do in difficult situations. And so we want to be able to continue to offer our programs. We travel the whole state, but we want to make sure we're in Logan County presenting our programs.
You're going to use your puppet for us?
Oh, you want to? See, this is why Sarah didn't want to have a puppet. She said, I'm going to have puppeteer. She said, they tried to make me use a puppet for you last night. My name is Steven, and I'm actually in the child abuse prevention program.
And I was abused. You might notice I have this scar here on my chin and on my arm.
That's because my mom, she used to abuse me. But I told my teacher, and I got the help that I needed. And my mom, she got to go to counseling, and she doesn't hurt me anymore. So this is just one of our characters. We have 22 different characters, and each character has its own story, its own struggle. And so the kids, when we're at school, can see themselves in the puppets, and our puppets are able to model what to do in different situations.

G-U-E-R-A. And so we would love to be able to do some programs here. I know last year you all, we want to thank you all for funding some programs for us, and we wanted to see if that's a potential again this year. Our programs, each, they cost $515 per program, and so we wanted to see if maybe you all would be interested in funding two programs for us.
I know we put some money in the budget to cover it for next year it's there I just don't remember the exact number but I know we have money in there every year that we support this program you guys did great work we're very proud of having you here I just don't remember the exact number of what we put in there I believe it's the two programs I think it is the two programs I think it's total of what we did last time

Never let you down. I know you've been thinner, but this is pretty thin over there. I don't see much of it all over there. But anyway, Friday, July 10th. 8 a.m. to 11 a.m. is our waste tire event. I'm expecting a good turnout, so I just wanted to let everyone know this is the county road department, 1080 Payton Street on the state site. 15 tire limit, passenger tires, and of those 15, two can be semi-tires. And I just want to remind, please don't cheat and try to bring too many tires. You have the husband bring a load and the wife bring a load, et cetera, et cetera. People do that. They do. So I appreciate that.
Wow that was the quickest I think we've ever gone through there so let's move forward to community announcements and because it's fair week and there's a lot of stuff going on it's going to take a few minutes to get through this so I'm glad you guys gave up your time for this Fair starts this week. June 23rd is fair fun with friends. Concession gates open at 5. $1,000 crash price drawing at 8 for the outdoor arena. June 24th is faith and family night. Everything begins at 5. Can you state your name?
Sure. Dr. Sam Lee. I appreciate the opportunity to be here. If you don't know me, that's probably a good thing. It means you've never had to come see me in the emergency room. My wife likes to joke when people say they met me, she says, I'm sorry. I'm never sure what to feel about that. So I'm the director of the medical center emergency room, and full disclosure, I'm also the medical director for ComCare, both here in Logan County and in Todd County as well. is required by the state of Kentucky and most states. It's designed to provide medical direction and education, but mostly it's so that EMS can legally acquire the medicines and drugs that they need on ambulances. And also to have me to help put out the proverbial fires that tend to come up. So I do appreciate the opportunity to be here. I also recognize and appreciate your all's efforts taking the matter of the EMS contract seriously and thoroughly. I know it's not an easy one. I know it's, I'm confident that your goal is not the cheapest route forward, but the route that confidently provides quality emergency medical care for our families and neighbors here in Logan County. I came here today to offer help. because i believe i have a unique perspective that can shed light on the ems services here in logan county both because of my various roles but also because of being a logan county citizen your examination of the ems services here started with public complaints long response times long wait times for transport out of the emergency room and they're merited they reflect a nationwide problem. There's a nationwide lack of crews, lack of trucks, lack of resources, and in some instances, all of the above. You see the same problem in medicine today. There's a nationwide lack of doctors. In particular, there's a nationwide shortage of radiologists. If you come to my ER, the most common complaint in the ER right now is having to wait for a report from an x-ray. So Logan County is not immune to these nationwide problems. But it's left the public and it's left you asking why are we waiting on an ambulance. It's left you with the question of how do we fix it and who should be responsible for fixing it. I think to fix that problem there has to be a full understanding of what the problem is. Part of my job as director is to give a direction that sometimes involves recognizing some blunt truths. I'm a little concerned that the public and in some instances political leadership fails to recognize how busy the emergency services in Logan County really is. That means EMS and my emergency room. And how busy we are is the root of the problem that you've been The Medical Center in Russellville emergency room treats 12,000 people a year, give or take. That's almost double the volume of the emergency rooms at the Medical Center in Albany, the Medical Center in Caverna, and the Medical Center in Scottsville. And that's about a third more than the volume of the Medical Center in Franklin. It's also a 20% increase in volume since the medical center acquired Logan Memorial two years ago. More patients in my ER means more EMS runs to the ER. It also means more EMS runs out of the ER, transferring those patients to different hospitals. As far as the EMS is concerned, in 2025, Logan County EMS made over 5,300 runs. When was this? In 2025. Over 5,300 runs. Our EMS crews are making more runs per truck than every surrounding county, with the exception of Warren County. And the argument can be made there that the majority of their runs are within the confines of the city limits of Oldenbury. We're also making more runs per truck than Christian County. More runs than Hopkinsville. We asked our crews to do this in the geographically seventh largest county in the state of Kentucky. 120 counties in Kentucky and we're the seventh geographically largest county. As it currently stands, this board is funding two advanced trucks for Logan County. Those same two trucks have been funded at nearly the same rate as when this board approved them 17 years ago in 2009. 2009 that was the year that george w bush left office gas was two dollars and forty cents a gallon stamps were 44 cents and that year logan county ems made 3 900 runs in 2009 made 3 200 3900 okay so here's the 2026 reality of what happens truck responds. A second 911 call goes out and the only other truck responds. Now when a third 911 call goes out, that patient waits for a truck. Or if that third call is more emergent than one of the first Or maybe you're in my ER and I've transferred you to the medical center in Bowling Green or a different hospital, and you're waiting for a truck to take you to that hospital. But now you have to wait until those runs are completed and a truck is free. Kentucky statute says that there has to be one truck in the county at all times. So now, now our crews are juggling. Where do they need to go? Maybe the patient would best be served at an outlying hospital out of the county, but because they have more runs waiting, they have to make a decision. We put on their shoulders the decision of what's best for the patient in the truck and what's best for the patient that's waiting. I believe the obvious answer is one to three more ambulances. in service but i recognize that ambulances aren't free more ambulances mean more crews and more crews mean more paychecks so like everything else your problem and your solution is money i started by validating the complaints and concerns from the lowland county citizens they are correct They see the problem. The problem is a consistently busy system stretched thin over the seventh largest geographical surface area in Kentucky. I'm here to tell you that there are three fixes to this problem. And not three possible fixes, but three concurrent and necessary requirements that I believe have to be in place before both you and I can look into the eyes of anybody in local county and tell them that help is on the way. Number one, at bare minimum, three trucks running 24-7. Two of those trucks need to be ALS, meaning advanced, 24-7. Number two, reimbursement needs to be competitive for quality paramedics. In no way am I saying that we don't have quality paramedics now. We do. But I have watched through the years as we have lost quality paramedics to other counties where they've been offered jobs making maybe the same, maybe more money, where they're asked to make less runs over a smaller geographic service area. And number three, and possibly the most important, is whoever holds the contract. You need to know their motivation. Is their motivation a stream of revenue? Or is their motivation the well-being of the Logan County citizens? Because to some people, they're patients. But to you and to me, their neighbors, and family. I urge you to look closely at the bids that you're receiving and ask if there's a Logan County connection. Nevertheless, honestly, it doesn't matter if the side of the ambulance says ComCare or Billy Bob's Ambulance Service. It doesn't matter. If those three things aren't met, If they aren't running additional trucks, if they don't value their crews, and if they don't have connections to Logan County, it's going to be more problems and more merit of complaints, and we'll be right back here next year. The amount of miles between Dunmore and Allensville doesn't get shorter with a cheaper contract. But it can be divided among more trucks. Miles equates to minutes, and minutes between a truck and a Logan County citizen having maybe the worst day of their life. I appreciate the opportunity to speak to you.
I have a question. You said at one point that the calls have increased immeasurably, excuse me, measurably, not immeasurably, measurably since the medical center took over Logan County. Give me a reason for that.
The medical center was a more respected name than Logan Memorial.
So once people called that, it was more what?
Just more calls? More people came to the ER. More people called the ambulance to go to our ER. I see that in my career I've seen that multiple times. That is not a statement against Logan Memorial or what it served as a hospital in no way. In community rural hospitals, the grass is always greener in the county next door. And the county next door has a big hospital system called the medical center.
That was my same question because I was really interested. How did population not change and your emergency room visits increased 20%?
Can you give me an approximate percentage of how many more calls? It doesn't have to be exact.
Since the medical center?
Since the medical center took over.
The volume in my ER is up about 20%. 20%.
I don't have that number.
We go up approximately anywhere from $75 to $150 a year, each year goes up. I know it's... Every year we just increase more and more.
It is $35.
But for the emergency room, that includes EMS arrivals as well as walk-ins.
But the calls themselves are 20% more now.
The arrivals to my ER, no matter how they arrive, is up by 20%.
Okay, I want to write that down.
Sure.
Which the number you gave makes sense. That's less than 2%.
You never can plan for when it's going to happen. I mean, you can have two serious calls and then you've got calls that I don't understand why people are calling an ambulance, but they're still calling an ambulance. We're constantly in need.
That's one of the hardest things is the unpredictability. Yes. Because you can have...
It's like, it's the same thing with our staff. You never know how many to have at a certain time. Usually you could say, okay, Friday's there, and the next thing you know, it might be Monday afternoon.
It's the same with the ER. I've had shifts where I have four ambulances and helicopters sitting in front of the hospital, and I've had shifts that I get to watch now. It's just the way it is.

although I do agree with about everything you said I do would like to make one remark the compensation side of that I agree with you 100% but you realize that we we pay a contract the compensation rate for their employees Comcare's employees is set by ownership not the county So even though I do think every one of those individuals sitting there needs to be paid more, you also have to realize that the county does not set that rate. No different than us asking a contractor who's doing paving to keep their employees at a certain level. That's not our responsibility. They give us a number. So when people come to us and ask us, tell us that they need higher pay, that's not on us. That's on the ownership of this ComCare. No, you can if you want to, but I think I just wanted to make that statement because I've heard it from others that it's on us, and it's not on us. We don't tell Scotties what they pay their people. I appreciate all the information. I do 100% agree with you on everything, but I just wanted that statement to be made. Is there anyone else that would like to speak?
I just want to say I do appreciate Dr. Lee coming. That's the most information that I've ever received here in this full meeting about the ambulance service and the responses and the increase. And, you know, I challenge any vendor, our current vendor or any vendor, who, as time goes on, to do what we just did. We're at the day of deciding on what views we're going with. And that's the first time that I've been in court eight years, and that's the first time that I know of that someone has came with those absolute statistical numbers to let us know that the problem, how the problem has progressed. So I just want to say I appreciate him letting us know all that information. And, you know, being on this committee, I've heard that information, you know, in the past. If no one else?
Let's move on to old business, which is the reason we're here. Discussed and once the service bids received the June 2nd, 2026 special called fiscal court meeting. We had a lot of committee meetings and we went over this fine a lot. We spent a lot of time working on this, trying to do what is best for Logan County. And I think as a committee, we made a 100%
agreement to the decision we've made Chris would you like to speak yeah just to give everybody full disclosure you know we did receive three bids I think you already know that different variables variations of those bids the ambulance committee which consists of sheriff Stratton Carl Sadler Phillip Ferguson myself Tyler and the judge I think we met four times two in person a couple times virtually we actually met and with John Holder to discuss their their bid and their just general concerns and questions we also met with some folks with AMR and had a good conversation with them as well all that said the committee is in full agreement of recommending the award of the next fiscal year contract to Comcare for their option 3B which was the one that they sort of defined themselves with the exception of the way it was worded it was worded in a way that the ambulance would be manned three twenty four hour ambulances would be manned by a paramedic or AEMT by request and in that conversation two of those ambulances are going to be manned by an ambulance 24-7 24-7 and one by an AEMT or paramedic on the third two of those will be manned by paramedic 24-7 and the third one will be paramedic or AEMT and that total price for that 3b option is five hundred thirty thousand five hundred dollars So that is the recommendation from the committee. And do you want to mention the other things as part of that?
Yeah, I think it would be appropriate. Certain things that the committee wanted to make sure were clear and transparent to ensure that we can have eyes on what's actually occurring and those terms were being met would be to put in certain stipulations that would be added into the contract. And I would suggest making our acceptance of their bid contingent upon those additions to the contract. we've been using roughly a very similar version of our existing contract with Comcare since I've been county attorney so I think it would be helpful for Comcare to know what they're getting and also for me drafting that for you to go ahead and list those terms that you want to make contingent that way if Comcare is unwilling to do that and then it would be a basis to go to the next bidder potentially
yes so uh i guess on behalf of the committee to put that form of a motion to accept uh the comcare bid 3b for a total of five hundred thirty thousand dollars five hundred dollars for three 24-hour advanced life support units with two of those units staffed by paramedic on each truck and the third one being staffed by paramedic or amt and that also being contingent on the contract including stipulations that monthly run volumes be supplied to the court that records of those on that's the word I'm looking for those workers on each shift a record of the shift work is also supplied so that we can verify that the paramedic and the AEMT requirements are met on each run and each shift I'm sorry not run each shift each shift has proper we're basically getting what William said which is the two paramedics yeah just to have verification and a check and balance that what what we asked for is being met thirdly this has been in the contract and I know John Holder has no qualms about offering this but just for public disclosure that Comcare continue to offer the training sessions for all of the of our dispatch or all of our first responders and whatnot uh those of just to reiterate what john said those have always been available but they're they have to be requested uh you know otherwise i know john they mentioned that he's offered those at times and then somebody not you know nobody show up for those so that's exactly right so make sure
He used to hold a regular call training and then several times. He did it for a while and then nobody showed up. So he started doing it on demand. I think it's probably John Craig. Yes, Craig.
so those are the those are the three you know sort of points of emphasis for the contract language that will then you know if this passes that will turn over to Joe to write up and include in that but obviously like I said check and balance of making sure that we're getting what we paid for especially considering if this passes it's a substantial increase and it's but it's also an expectation of even better service and I mean when I say better advanced service so that's the motion and recommendation of the committee unless I left anything out I did not.
All I would have to say in addition to that is, you know, like I said, I'm part of this committee. I can talk all this stuff that is good education for us. And it's clear from talking to people we've talked to and looked at other counties, it's obviously clear that we need to go into the preambulances. Um, that is very evident. And, um, you know, just by looking at other, even at the last conference I was at, when I was talking to another neighboring county that has a 12,000 population, that has two full-time ambulances in the part-time. So, I mean, Denali County has three ambulances. Simpson County has three ambulances. You know, they have half the mileage we have, and they also have 20,000 population-ish where we have close to 30,000 population. So it's clear that we need free ambulances full-time to serve our community. Now, as part of ComCare, you know, the community is recommending ComCare. So, you know, we're going, we're asking to go to two to three ambulances. And the cost is increasing, I think, around 125%, right? So the cost increases 125%, like Chris said. We don't bill people directly. Our charge is $675. The other two bidders, $675. The other two bidders, over $2,000. The next one's over $3,000. you want to tell me i need to pay everybody i want to my gosh people are worth way more than they pay and you're correct and dr lee's correct they do deserve a higher pay rate and you're absolutely correct we set that rate how what do i pay them with i pay them with the subsidy that i'm asking so i take your point but when we raise our subsidy a little bit and we We usually raise it at that cost of living, 3%, 4%, something like that. Right. And usually you vote on it. Sometimes it's a correction of 4% to create pay that's going up. I can't pay my people a whole bunch more money and not increase it.
And I agree with that 100%. But what I'm saying is I have – others in the community calling me complaining that you're not paying them enough and I have to say I'm not in charge of what you pay your employees that was my whole point of that whole spill is I don't set that I can't set a rate higher than what I have in the bank

As you mentioned, yours is $675,000. The next closest out of all who bid was over triple.
The difference is they can charge what they want, they can pay that. CMS sets my rate. is what we get for a Medicare, Medicaid loan. We lose money every time we turn a wheel. So when we say subsidy, we're losing money doing what we're doing. I can't charge more like they can. I get what I get. we're gonna have to get together and you know after this and you know hopefully the committee will continue to work and

okay now going back to the critical question asked by Magister Bolden the honest truth is is this emergency funding has been kicked down the road for years it's been done being done with the EOC we've been subsidizing it with occupational forever once a landline started dropping that's when we started losing funding and it's been like I said it's been kicked down the road for years and it's been something that's needed to be addressed for years and it's something that we can't take lightly we have to address this in the next year so whoever is sitting in this seat or sitting at those seats has to address that My thought is pulling out of the ARPA money to subsidize this this first year, but we actually, as a court, we have to work on figuring out a funding source for our 911 services. We're one of the few counties in the state that does not have a basic taxing district for emergency services and ambulances, and that's something we have to look at.

In the 20 July... the 26-27 budget we have put money in to do you see to provide tango tango which is a radio backup system through the fire department sheriff's office EMS's phone systems or phones pardon me I just the sheriff was just going to be asking for

law enforcement, EMS, search and rescue, and I think we were still kind of involved in how to, because some of them can edit their departments together on one channel, like search and rescue, and emergency management are pretty much kind of one thing, so it doesn't leave much out. And then what that does is creates, they can, fire departments can create subgroups that will, they will be able to communicate with each other There is no backup system for that at all. So this is the backup? So it's a relatively inexpensive backup. Is this year? Lots of cell phone coverage, which gives us better coverage actually.
So we're going to enter a three-year contract? That's what it says.
I mean is this something that you see recurring indefinitely yeah I mean I mean again if you you do it one year the next year you don't you know you're gonna have to do a test just to see now we've been testing it for probably six or eight months we've some of the fire chiefs have been utilizing it seeing how it works a lot of the fire departments are paying for active 9-1-1 which is basically a cab gets sent when creating for their runs to the phone or when they page out it also creates an alert with this there's no they don't have to pay that no more so we've been utilizing it for about six or eight months and if the fire departments decide that they want to stay with act 9-1-1 that's their choice but it creates so like every time my guys do a traffic stop It seems to be working on the details of whether or not what you're doing.
